👂耳のエピテーゼ シリコン

正確にはシリコーンと書くべきなのかな?
シリコンと書くと硬い方の物質のシリコンになるんだったと思う。

まあ細かいことは気にせずに、今回もスマホから書き込んでます。

通常写真撮るのもこのスマホだし、画像の転送をする必要がないのでその点は楽。

あと絵文字もちゃんと表示されるようなので。

例の耳のエピテーゼをシリコンに置き換えたところです。

色合わせを今回はつめてやってみたので、今日の試適とステインが少し楽しみです。

結果が良ければ、色合わせのステップが確立されたことになりそうです。

写真でお分かりのように耳の形状で深くくぼむべきところにスキンタグが位置しています。

この部分をなるべく目立たなくする方法も多分ほぼ確立されたはず。

このあたりはまた別の機会になりますね。

いつも思うのですが、人体はどこをとっても滑らかで美しい曲面で構成されています。

これを一からワックスアップで模倣するのはやはり大変です。

3Dを活用するいまの方法だとこの部分もずいぶん楽になります。

多分エピテーゼの作り方 その2にこのあたり全て詳しく載せようと思っています。

また後ほど試適とステインの結果をアップいたします。多分、、、

Wax pattern from digital analyzing

I uploaded the video on Youtube last time.

I was thinking about the process of analyzing and designing the epithesis in 3D, and then finishing it up.





I wish I could print it with silicon or wax on my 3D printer, but I can't do it with mine.

The final positioning and fusioned with the skin tag was printed with resin.

If I put this mold onto the skin tag model, it provides space for prosthesis.

The question is whether the patient will be satisfied by trying this on the actual area.

The skin tags are slightly lower and more forward.

I've adjusted the size of the wax a little to take that into account, but what do you think?

Bilateral microtia case by 3D technology

Sometime I have cases of bilateral microtia.

Many cases of microtia are unilateral, so usually I need to copy the shape of normal side.

Since there is no original shape for both ears, I can make it by imagining the shape of the ear for each face, gender, age, etc., or I can make it from the shape of the ear if it resembles a parent or brother.

This time I did with using 3D printer.

Here's how to do it with a 3D printer.

It used shape from a 3D printed ear, the advantages of this method are

1. Easy to transform, enlarge, and reduce shapes
2. Flip right and left easily.

These two points are the most beneficial.

Of course, the data of the original ear shape must be prepared from the scan data.

The case is a 18 year-old man with skin tags on both sides that are much lower than normal.
The hairline is also below.

It's a case to wear with medical adhesive, so I can't make epithe beyond the hairline.

As usual, I mold the skin tag part with alginate impression material.IMG_4084

In the usual case, the skin tag is epitomized, but in this case, epitese might be lower and bigger as an natural looks.

From the previous digital data of the ears, I printed the left and right ear pieces at 90% of the average adult male ear size.

I decided not to cover the skin tag completely, but to expose it as an epithese earlobe.
Therefore, it is necessary to make it so that the overall shape does not become unnatural.

In addition, the shape of the skin tag is quite different between right and left, so I need to be a little creative here.

3D printed left and right ear shapes. This is resin, so I duplicate this with alginate impression material and pour melted wax.

Then, I will try the wax pattern next time and check the final shape.

The digital data I used this time was a 3D scan of another ear.
The shape of the earlobe was voluminous, so it was convenient.
